These centers offer similar programs as the social model Day Center (above) but also provide various levels of care for individuals with disabilities. Programs include physical, social, and recreational programs, personal hygiene assistance, and meals and snacks. ADHCs also focus on multi-disciplinary team assessments, medical and nursing services, and provide such special services as Physical Therapy, Occupational Therapy, counseling and support services, social services, health screenings, family support and more. Transportation may also be provided.
